The database provides a comprehensive facility for managing health-related questionnaire responses from many patients. Many questionnaires are available which concentrate on specific areas of health and patients can complete those which are relevant to their symptoms.

The database can store completed questionnaires from many patients. Questionnaire responses can be entered directly by the patient, or questionnaires can be printed and the responses collected from paper. This can be done over a period of time and further questionnaires can be selected and completed as further information is found from the patient.

The database can analyze all of the questionnaire responses for each patient and suggest relative priorities for different categories of nutritional support.

Personal History Questionnaire:

The personal history questionnaire gives a general overview of the patient’s health and is recommended for every new patient. It is a tool to provide you with information about the patient, which will direct you to more specific, detailed questionnaires. The database can generate a graphical interpretation of the personal history questionnaire and this will identify other questionnaires that will be useful in evaluating the patient’s health and nutritional needs.

Detoxification Questionnaire:

The detoxification questionnaire provides information about the patient’s ability to remove toxins from the body. This questionnaire is highly recommended for anyone with chronic health problems.

Gastrointestinal Questionnaire:

The gastrointestinal questionnaire provides information about the patient’s ability to digest, absorb and assimilate nutrients from our food. This questionnaire is highly recommended for anyone with digestive problems. Each section of this questionnaire evaluates different gastrointestinal functions. For instance:
